چکیده

set included WCC (< 4, 12–19.9, or ≥20 × 10/L), BUN≥11 mmol/L, serum albumin <25 g/L, heart rate > 90/minute, and respiratory rate >20/minute. A predictive score of min 0 and max 13 points was derived. A score ≥7 points was associated with 70% cases of cCDI, showed 68% sensitivity (95% CI, 55–80) in the derivation set and 70% (51–88) in the validation set, a specificity of 73% (69–76) and 76% (72–81) respectively, 17% PPV (9–25), and 97% NPV (95–99) in both sets. Conclusion. Using a large multicenter prospective cohort and robust modeling approach, we derived a predictive score that included easily available measures at the bedside. The score showed acceptable performance. Further validation is needed on cohorts with different characterstics (non-outbreak setting, higher rate of cCDI). Other approaches such as combination of biomarkers could be more predictive of cCDI. Disclosures. J. A Multi-Center Prospective Derivation and Validation of a Clinical Prediction Tool for Severe Clostridium difficile Infection

BACKGROUND AND AIMS Prediction of severe clinical outcomes in Clostridium difficile infection (CDI) is important to inform management decisions for optimum patient care. Currently, treatment recommendations for CDI vary based on disease severity but validated methods to predict severe disease are lacking. The aim of the study was to derive and validate a clinical prediction tool for severe outc...

Injudicious antibiotic use leading to fulminating Clostridium difficile infection: a case report

The case illustrates the myriad of fulminating complications due to Clostridium difficile infection in a previously healthy individual without any risk factors. Community acquired Clostridium difficile infection can occur even many weeks after the course of broad spectrum antibiotics. There is no definitive pattern or guidelines to predict who would develop the fulminating complications.

Derivation and validation of a simple clinical bedside score (ATLAS) for Clostridium difficile infection which predicts response to therapy

BACKGROUND Clostridium difficile infection (CDI) continues to be a frequent and potentially severe infection. There is currently no validated clinical tool for use at the time of CDI diagnosis to categorize patients in order to predict response to therapy. METHODS Six clinical and laboratory variables, measured at the time of CDI diagnosis, were combined in order to assess their correlation w...

مروری بر مطالعات اپیدمیولوژیک عفونت‌های ناشی از کلستریدیوم دیفیسیل

Clostridium difficile (C. difficile) is an important factor in the development of the gastrointestinal diseases because of irrational antibiotic prescription and antimicrobial resistance. In the past, this bacterium was introduced as an agent of the infection in the hospitals called "hospital acquired Clostridium difficile infection". This infection is a main cause of morbidity and mortality in...
